c0nfused88
I am having surgery tomorrow and think I will have to fill these forms out. What exactly does this entail? Does the person I give it to have to be there? Would love to hear any of your experiences with this.
CarrieT
Powers of Attorney have to be notarized and can be general or specific; you define what powers you are giving another.
DianaLorens
Power of attorney is legal document that allows you to name a person/persons to make certain decisions for you. This document becomes effective when you sign it. This legal document must be provided prior the day of surgery and A legal guardian must be in the center AT ALL TIMES for children under 19 years and dependent adults. I hope all goes well with your surgery
